Employee Notification for 1095-C
According to the Paperwork Burden Reduction Act, employers are no longer required to automatically mail Form 1095-C to employees.
The 1095-C will be available for our employees to view and print through Qmlativ Employee Access no later than March 3rd.
Printed 1095-C forms will only be provided to employees upon written request sent to Julie Carlson at Julie.carlson@ferndalesd.org.

- Employees are not required to make any changes to their health plans during open enrollment unless their health plan is no longer available
- Employees who do not make any changes will remain enrolled in their current health plan.
- Employees who choose to participate in the Medical FSA or DCAP program MUST enroll or re-enroll every year.

3. Employees may:
a. Enroll in or change medical , dental and vision plans.
b. Add or remove dependents from medical, dental and vision plans (with dependent verification documents required).
c. Waive medical coverage.
d. Change employee-paid long term disability.
